Disqualification Procedures
If a player, coach or sideline personnel is ejected or Disqualified (DQ) for language, fighting or any infraction covered by the sportsmanship statement then you must report this.
- Once an ejection is declared, calm the field. This includes the officials: having an ejection is rare, but it certainly can become emotional. Remain the calm force on the field.
- Go to the scorebook and get the names of the player(s) ejected. This is vital.
- AS SOON AS POSSIBLE: talk to your partner. Make sure you have the story and situation straight as to what happened.
- Contact Derek, one of the committee members, or another member of the board to go over your report.
- Go to the NJSIAA website https://www.njsiaa.org/form/disqualification-report
- Sign in under your login and password
- Follow the prompts and fill in the information requested
- This needs to be done within 48 hours of the DQ.
- You will receive an email confirmation of your report. IMMEDIATELY forward this to Chapter Secretary Dave Frick and Derek Pukash.
When writing your report, make sure you write exactly what happened but DO NOT editorialize (“Red player number 12 was being a jerk the whole game and then he sad…”)
Instead: Red player number 12 told me to go f myself and I ejected him 4Q 2:08.” “ Blue player number 14 threw a punch at Red player and then the Blue coach called me an a-hole so I ejected them both 2Q 4:20.
**You need to write it exactly how it happened.**
Remember this: a player ejected is a done deal; you can’t put the toothpaste back in the tube. Ensure 2 things: first, that it is an ejectable offense; second, that you have the correct numbers and names.
If a player receives 5 minutes of personal penalty time, they have fouled out and there is no need to fill out a DQ form.

Responsibilities of the Ejecting Official (Referee, Umpire, or Field Judge):
- Contact Derek Pukash to inform him of the ejection.
- Call the School Principal by noon the next day after a disqualification.
- Fill out the online NJSIAA DQ form.
Filling out the NJSIAA DQ Form:
- Go to www.njsiaa.org and Log In by clicking on the Green Tab: Schools, Officials, Media
- Enter the user name and password.
- Then go to “Officials” (blue tab) and the drop down menu: “Disqualification Form”
- Click on and begin entering the disqualification information.
- Be sure to enter every field.
- Be certain you add in your email address in the bottom field
- If you want to add the officials who worked the game with you, add them at the end of the explanation section.
- You must fill out one form for EACH player/coach ejected. You cannot put two players on the same ejection form.
- Once you hit submit, you will get a confirmation that the DQ has been submitted.
- You will get an e-mail confirmation that you should forward to David Frick (davidfrick@njiloa.org) and Derek Pukash.
The high school Principal of the offending school will receive an e-mail confirmation.
The submission of the form must be completed online with this format to keep the NJSIAA data base accurate and notifications prompt. Any issues may be directed to tbennett@njsiaa.org or kcole@njsiaa.org and we will assist during this transition time.
This form must be completed within 72 hours of the DQ.
Reminder: OFFICIALS MUST NOTIFY THE SCHOOL WITH A PHONE CALL, FIRST AND FOREMOST ASAP AFTER THE CONTEST IF A DQ OCCURRED (by Noon the next day).”
Unsportsmanlike Conduct Form
- Go to www.njsiaa.org and Log In by clicking on the Green Tab: Schools, Officials, click on Unsportmanslike Report
- Be sure to enter every field.
- Click submit at the bottom to submit the form.
